Patient Consent Form (GDPR regulations)
As registered classical homeopath I am obliged by law to keep patient medical records for at least 7 years following the last consultation on which treatment was given. In the case of minors, it is advisable that the records are kept at least 7 years after they reach majority (18 years of age).
Patient data and records are digital and will be kept in password protected files on both a hard drive and on the cloud for the above periods of time. I use effective virus protection and firewall software.
The patient’s paper files are kept in a designated location in my office, secured from external access. At no time will patient data/records be passed on to any third party without your consent.
By booking your first appointment you consent for your records to be kept at my office under the above terms.
